How is this different from PDF to image?
PDF to image exports the final appearance of each page. Image extraction exports the original embedded images—photos, screenshots and graphic assets—used inside those pages, rather than manufacturing page screenshots.
How does it avoid needless recompression?
Reusable JPEG and JPEG 2000 streams retain their original compressed bytes. Resources that need colour interpretation or mask composition are exported as lossless PNG.

Yes when the scan is stored as an image resource; vector pages are not turned into manufactured screenshots.
Will JPEG quality change?
